Use this form to request the deletion of your 4D Prom account and associated data.
Deleting an account is not instant. Because 4D Prom stores clinical questionnaire (PROM) data that may need to be retained for legal or research reasons, deletion requests are reviewed and processed manually by our team, typically within a few business days.
